How do I change my name on my life insurance policy?
Complete the Change of Name section of a
Policy Change Request Form and mail the completed and signed form to
our Customer Service Center.
You must include proper documentation for all name changes other than by
marriage or divorce. The change cannot be processed without such proof.
We will send confirmation of the name change to you at your address of record.
